JOB SUMMARY: The Clinical Coordinator will coordinate the oversight of all mental health assessments and supports provided to re-housing and stabilization services for families within the Family Services Branch. This includes families living at the Family Life Center, the David Jon Louison, Bolton Place Family Center and community families that have been re-housed. This position is responsible for ensuring that a high level of service is provided to all children and families in all program areas within Family Services. Act as part of administrative team with Assistant Directors, Lead Clinician, Director, Executive Director and Vice President and perform duties within Family Services Branch as needed to ensure appropriate service delivery. The Clinical Coordinator will accept and display the values of caring, honesty, respect, and responsibility, as well as the values of the Developmental Assets, Family Strengthening and Trauma Informed Care consistent with the Old Colony Y. JOB FUNCTIONS:

  • Lead the coordination of clinical services provided to current and graduated Family Services families and their children. This includes families that are within the community that are "Stabilization Families," in need of on-going case management and support services.
  • Collaborate with the Lead Clinician, Assistant Directors and Director to identify staff training needs and to facilitate training programs and staff supports.
  • Facilitate individual supervision, clinical team meetings and case management reviews with designated staff as needed and in collaboration with Lead Clinician.
  • Maintain a caseload of families to meet the needs of the Branch.
  • Meet on-call needs of program. Respond to emergency situations as needed. May include evening, weekend and holiday hours if circumstances warrant.
  • Maintain communication with DHCD, HUD, and other state and federal agencies, schools and other human service agencies to include: Intake and assessment, referrals to other human service agencies, coordination of services, and family focused care.
  • Accompany families to clinical meetings or appointments as needed.
  • Conduct assessments, create safety plans, and work in collaboration with Family Services staff to develop individual support plans that recognize the uniqueness and interest of the family and work with team to ensure supports are in place.
  • Act as a liaison to family and/or support systems. Assist staff in facilitating ongoing communication with collateral contacts as needed.
  • Ensure accurate record keeping including: Client records, approval of correspondence with outside agencies, program billing, and appropriate outcome evaluation.

  • Consult with collateral and clinical staff who will provide direct support to families and provide needed information to Family Services staff to improve care and wrap around support.
  • Collaborate with program staff to assist families with their needs, overcome barriers, and assist with developing self-sufficiency goals
  • Conduct outreach and survey communities to identify additional clinically supportive resources for families.
  • Maintain regular contact with the Lead Clinician and Case Managers to assess the family's progress in meeting its clinical needs and to address any barriers to obtaining said services.
  • Participate as part of management team of the Family Services Branch and assist with daily operations of the Family Life Center, David Jon Louison and Bolton Place as needed.
  • Develop and maintain a warm, welcoming environment within all centers by supporting staff and families in a positive, strength based manner.
  • Maintain up-to-date client records on HMIS database and in locked file cabinet.
  • Participate in staff meetings, weekly clinical meetings, community meetings and trainings as requested by the Director, Executive Director or Vice President.

    • Master's in Social Work, Counseling, Special Education, Psychology, Child Psychology, or a related field is mandatory; LMHC or LICSW required.
    • Minimum of five years of related work experience.
    • Excellent oral and written presentation skills and demonstrates professionalism with staff, parents and participants.
    • Experience with in-crisis youth and families.
    • Prior success with non-profit housing organizations and community collaborations.
    • Prior success with assisting individuals secure affordable, permanent housing preferred.
    • Experience with Microsoft Office, including Word, Excel, Access and PowerPoint.
    • Additional skills should include correct grammar, punctuation, spelling and business writing
    • Ability to lift 50 lbs.
    • Must be able to sit down, stand up, walk up and down stairs and bend over during the course of the day.
    • Must be an insured and licensed driver able to travel for occasional home visits, transportation of clients, and provider meetings, as well as able to pass pre-placement drug screen. -
