Job Description:

Victory Human Services must have staff members who provide oversight and monitoring to the caregiver and meet the following qualifications and training requirements:

The AFC provider must provide nursing oversight by a registered nurse or licensed practical nurse under the supervision of a registered nurse who meets the qualifications as described in130 CMR 408.433(C)(2)(a) and 130 CMR 408.433(E)(a), who is not related to the member, and who is licensed in Massachusetts.

The responsibilities of the License Practical Nurse include:
Complete the MDS-HC or CDS and other nursing assessments and coordinate all other applicable clinical assessments
1. Develop and review on an ongoing basis each member's AFC plan of care.
2. select, train, evaluate, and supervise AFC caregivers in conjunction with the care manager.
3. Work independently and collaboratively with RNS, CMs, CHWs, Office Personnel and the AFC management team.
4. Complete annual assessments, six-month summaries and MDS-HC when necessary.
5. Review the physician summary form.
6. Create, update medical logs, request and review medical records from the MDs.
7. Monitor the health status of all members to ensure that all needed AFC is professionally delivered.
8. Report changes in the health status of any member to the member's physician.
9. review member's medical documentations and report medications and diagnoses as necessary in member's profile.
10. Conduct on-site visits with each member at the qualified setting to determine level of care:
11. Complete assessment as level I when member has two or less physical assistances on PCP order form.
12. Complete assessment as level II when member demonstrates the need for three physical assistances or more.
13. Complete nursing progress notes corresponding with each on-site visit or encounter, or more often as the member's condition warrants.
14. Plan for and implement discharges from the AFC program.
15. Conduct an orientation for each AFC caregiver before the AFC caregiver begins personal care.
16. Provide ongoing training to AFC caregivers on health and aging.
17. Submit intake assessments to insurance payer, as necessary.
18. Provide any additional nursing support when situations warrant.
