Job Description:
Encompass Health Corporation is seeking a dedicated Case
Management Director to lead our comprehensive case management
strategies across our network of inpatient rehabilitation
hospitals, home health agencies, and hospice agencies. The
successful candidate will be responsible for overseeing patient
care coordination, optimizing patient outcomes, and ensuring
exceptional service delivery in line with regulatory compliance.
This role involves strategic planning, team leadership, and
collaboration with various healthcare professionals to maintain the
highest standards of healthcare services. Develop and implement
case management policies and procedures to ensure quality patient
care across all facilities. Lead a team of case managers and
support staff, providing training and development to enhance
performance. Collaborate with healthcare providers, patients, and
family members to ensure cohesive and comprehensive care plans.
Monitor patient care processes and outcomes, utilizing data-driven
approaches to enhance service delivery and patient satisfaction.
Ensure compliance with local, state, and federal regulations
concerning healthcare case management. Manage budgetary allocations
for case management activities, optimizing resource use and
operational efficiency. Stay updated with the latest trends and
advancements in case management and healthcare regulations.
Excellent leadership and organizational skills Strong communication
and interpersonal skills Ability to handle multiple tasks and
maintain calm under pressure Expertise in regulatory healthcare
standards and compliance requirements Proficiency in healthcare
management software and patient databases Medical Insurance Dental
