Important Requirements: /t - RN needs to have 1 year Home Health Hospice experience in last 3 years /t - HCHB charting required Pay: /t - $2,228.00 (Travel pay package) /t - Local pay package available /t - Mileage reimbursement for patient related travel Work Schedule: /t - 8:00am - 5:00pm (Monday to Friday) /t - 40 hours per week /t - Territory: Peterboro and Milford area Contract Details: /t - 13 week contract (possible extension) /t - Nurses are paid by the hour, not by the visit Job Details /t - Perform comprehensive assessments and develop individualized hospice care plans /t - Deliver skilled nursing care, symptom management, and emotional support /t - Educate patients and families on hospice philosophy and comfort care practices /t - Collaborate with the interdisciplinary team to review and adjust care plans /t - Maintain accurate documentation in compliance with hospice standards Qualifications /t - Current RN License /t - Must have at least 12 months of home health hospice experience within the past 3 years. /t - OASIS experience /t - Hands-on HCHB experience required /t - Current driver's license /t - American Heart Association BLS KMG Healthcare Job ID 31775010.
