Job Description Job Description The VNA & Hospice of the
Southwest Region is looking for a per diem Spiritual Care
Coordinator/Chaplain to serve our patients in Franklin County. This
position will be responsible for developing an ongoing plan of care
for the spiritual needs of Hospice patients and families, assisting
them with end-of-life concerns and coordinating resources to meet

HomeCare Elite. Key Responsibilities: Plans and implements a
variety of activities for the Hospice of VNAHSWR spiritual services
program and the delivery of spiritual services to
patients/families. Works collaboratively with VNAHSWR staff, clergy
and community groups to enhance their sensitivity to the spiritual
concerns of patients/families experiencing terminal illness and
loss. Takes action to ensure that information is provided in a
timely manner to maximize the positive impact on the
patient/family. Participates in patient conferences by exploring
and assessing the potential spiritual needs and bereavement
counseling of patients/families/caregivers and providing/reporting
on services as indicated. Proactively liaises with clergy or other
appropriate spiritual representatives of patients/families as
needed to maximize effectiveness of spiritual care continuum.
Serves as an advocate for patient and family members with regard to
spiritual needs. Makes recommendations to IDT for needed services.
Actively develops educational programs and materials for
patients/families, staff and the community on spiritual and
end-of-life issues. Works collaboratively with Bereavement
Specialist to maximize continuum of care as clients/families move
the various stages of grief. QUALIFICATIONS: Graduate of accredited
seminary or school of theology or appropriate certification in
hospital or pastoral ministry. Demonstrated experience in working
with patients/families dealing with life threatening illness and
death. Valid state driver's license required. Note: Passing a
pre-employment background check, reference check and pre-employment
physical are required for employment with VNA & Hospice of the
