Job Description:
General Statement of Duties: Provides support and advocacy to
parents, guardians, and caregivers of FEP program youth and ensures
the inclusion of family voice throughout the care planning process.
Serve as role model for parents and caregivers in interacting with
team members and community providers and navigating the state
mental health system of care. Must have personal experience as a
parent/guardian/caregiver of a youth/family with mental health
needs and experience in navigating any of the child/family-serving
mental health systems. BAMSI is a private, non-profit culturally

Details related to demonstrating compliance with this requirement
will be provided to candidates who have been selected for
employment.+ Participate in DMH hand off and intake of new families
in a timely and family-friendly manner. + Provide direct guidance,
coaching, training, support, education, and advocacy to up to 10
families.+ Assist families in how they can appropriately articulate
strengths, needs, and future goals.+ Support families with regard
to relating with their child and networking with community
resources providers and schools to meet the individual needs of
youth.+ Conduct a comprehensive home-based assessment and support
family through the development of vision statement. + Engage
families with activities in the home and community that are
designed to address one or more care plan goal(s). + Assist
families in recognizing the importance of self-care as a vital
component of their parenting. + Attend meetings with families and
support the team with discharge and/or transition planning.+
Participate in and/or facilitate care plan meetings to model
skillset for families.+ Conducts a minimum of weekly face-to-face
home visits with families and telephone support to families as
needed.+ Organizes and co-facilitates a monthly peer-to-peer
support group to provide education, coaching, resource development,
and networking opportunities for FEP families. + Utilize personal
journey in support of parent/guardian growth and development.+
Attends and actively participates in weekly supervision, training,
coaching, and team meetings as assigned.+ Complete weekly progress
notes, establish care plan goals, contributes to the development of
the comprehensive home-based assessment (strengths section) and
transition summaries.+ In collaboration with the family team,
develop, implement, and monitor a youth/family-driven care plan.+
Track direct/non-direct service hours and provide weekly report to
supervisor.+ Establish and maintain collaborative relationships
with community providers, community organizations, state agencies,
CSAs, and other programs within the BH division to ensure a
comprehensive and consistent treatment approach.+ File a 51A with
support from clinician and supervisor as indicated.+ Maintain
regular and timely attendance high professional standards at all
times. Perform all duties consistent with BAMSI/DMH policies and
procedures. Execute duties to reflect reasonable safety standards.
Universal/standard precautions must be utilized and training
obtained in areas that constitute risk. Perform other related work
duties as needed or as assigned by supervisor and/or designee+
Associates degree in a human service field from an accredited
school and one (1) year of experience working with SED families; or
High school diploma or GED and a minimum of two (2) years of
experience working with SED families.+ Must have personal
experience as a parent/guardian/caregiver of a youth/family with
mental health needs and experience in navigating any of the
child/family-serving mental health systems+ knowledge of meeting
facilitation required. + Experience working in an environment that
is youth and family driven and must have knowledge of community
resources in designated work area and awareness of culturally
relevant services.+ The ability to speak one or more languages
(Cape Verdean, Haitian Creole, and Spanish) preferred.+ Ability to
communicate effectively with service providers and administrators,
to work independently and as part of a team and embrace the
wraparound process and systems of care principles. + Broad
understanding of DMH mandated regulations and policies and CBHI. +
Strong administrative skills, knowledge of MS Office, navigation of
electronic health records, written/oral communication,
detail-oriented, and ability to flex work schedule to meet the
needs of youth and families as necessary.+ Valid drivers license,
own means of transportation, and willingness to travel at least 60%
