Job Description:
The Department of Developmental Services (DDS) is dedicated to
creating, in partnership with others, innovative and genuine
opportunities for individuals with intellectual disabilities to

communities as valued members. DDS is seeking a motivated and
experienced individual to provide supervision and leadership as a
Human Service Coordinator II (Service Coordinator Supervisor) at
the Plymouth Area Office in Plymouth, MA. Job Responsibilities:*
(the duties and responsibilities stated are general summary and not
all inclusive.) The Plymouth Area Office in the Department of
Developmental Services (DDS) is seeking a professional candidate to
perform the duties of the Human Service Coordinator II (HSC II).
The HSC II provides direct supervision to each assigned Human
Service Coordinator I (HSC I), including assigning cases as needed,
providing support to HSC I's for crisis management, conflict
resolution, facilitating Individual Service Plan (ISP) development,
Meditech and Home and Community Services Information System (HCSIS)
management, and the coordination of supports for the individuals
supported. The HSC II ensures that each component of the ISP
process and/or otherwise contracted services are completed for each
individual receiving supports from the Department per DDS practice
and regulations. The selected candidate responds to urgent requests
and completes routine referrals to area programs and services for
individuals. This position also provides support and consultation
to local area provider agencies, family members, and other
stakeholders, including assisting Service Coordinators to
facilitate cooperative communication and resolution to critical
issues. The HSC II works with the Area Director and Assistant Area
Director to maintain automated data systems, including compiling
specific statistics as needed, in addition to managing related
projects within the Area Office as assigned. This critical position
is part of the Area Office Management Team, participating in
provider meetings, planning for services, and working to address
and manage risk for individuals served in conjunction with HSC I's
and the Area Office Management Team. Preferred Qualifications: *
Experience in working with families, providers and other
stakeholders within the DDS community. * Experience supervising
high performing teams. * Strong clinical and risk management
skills, with sound judgement. * Familiarity with use of the
Meditech program and the Home and Community Services Information
System (HCSIS). * Competency in basic computer skills (word
processing, data base management, and e-mail). * Great ability to
communicate well with others and work within a team context.
Schedule: Full-time - 37.5 hour weekly position. Schedule - Monday
through Friday 8:45 AM- 5:00 PM Days off - Saturday and Sunday * *

may be occasional nights and weekend meetings with individuals,

motor vehicle driver's license at a class level specific to
assignment. Substitutions: I. A Bachelor's degree or higher in a
related field may substitute for two (2) years of the required (A)
experience. Based on assignment as Qualified Intellectual Disabled
Professionals, within the Department of Developmental Services, a
Bachelor's degree or higher in social work, psychology, sociology,
counseling, counseling education, education of the physically or
emotionally handicapped, education of the multiple handicapped,
education of the learning disabled, human services, rehabilitation,
rehabilitation counseling, nursing, recreation therapy, art
therapy, dance therapy, music therapy, physical education or other
a related field is required. Based on assignment as a Mental Health
Case Manager or Forensic Transition Team Coordinator, within the
Department of Mental Health, a Bachelor's degree or higher in
social work, psychology, sociology, counseling, counseling
education, education of the physically or emotionally handicapped,
education of the multiple handicapped, education of the learning
disabled, human services, rehabilitation counseling, education or
